By using our website or telehealth services, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree, please do not use our services.

2. Services Description

RemoteHCS provides remote patient monitoring (RPM) services, including but not limited to:

• Chronic Condition Management, for diabetes, high blood pressure, heart conditions, and chronic respiratory conditions
• Post-Operative Recovery Monitoring
• Medication Adherence Tracking
• Vital Signs Monitoring, including blood pressure, heart rate, oxygen saturation, and weight
• Care Team Communication

3. Telehealth Limitations

Telehealth services are a convenient alternative to in-person care but have inherent limitations:

• Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 immediately.
• Some conditions require in-person physical examination, diagnostic testing, or emergency intervention.
• Our providers reserve the right to recommend in-person care if telehealth is deemed insufficient for your condition.

4. User Responsibilities

As a user of our services, you agree to:

• Provide accurate, current, and complete health and contact information.
• Maintain the confidentiality of any account credentials.
• Use the services only for lawful purposes.
• Not record telehealth consultations without explicit written consent from all participants.

5. Text Messaging Program

RemoteHCS sends text messages about appointment scheduling and reminders, care coordination, and replies to inquiries, to people who have opted in to receive them.

• You may opt in one of two ways: verbally, during a phone call with a care coordinator, or by texting the word START to 855-541-9891. There is no online form on this website for signing up to receive text messages.
• Message frequency varies.
• Message and data rates may apply from your mobile carrier.
• Reply STOP to cancel at any time. Reply HELP for help. You can also reach us by phone at 855-574-4200.
• Mobile carriers are not liable for delayed or undelivered messages.
• Consent to receive text messages is not a condition of receiving care or purchasing any service from RemoteHCS.

6. Intellectual Property

All content on this website (including text, graphics, logos, images, and software) is the property of RemoteHCS or its content suppliers and is protected by copyright, trademark, and other intellectual property laws.

7. Limitation of Liability

To the maximum extent permitted by law, RemoteHCS shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of, or inability to use, our website or services.
